Exploration des pratiques infirmières de suivi de deuil auprès des familles en CHSLD : une revue narrative des écrits

Les centres d’hébergement et de soins de longue durée (CHSLD) sont des milieux de vie qui accompagnent non seulement les résidents, mais aussi leurs proches, et ce, tout au long du séjour du résident. Ce soutien devrait se prolonger après le décès, soit dans l’accompagnement du deuil de la famille. Le deuil peut avoir des répercussions importantes sur la qualité de vie des familles, notamment lorsqu’il évolue vers un deuil complexe persistant. C’est pourquoi l’accompagnement des professionnels de la santé, dont les infirmières, est essentiel dans cette transition importante. Or, les pratiques actuelles de suivi de deuil en CHSLD sont plutôt limitées et se traduisent principalement par des gestes à visée commémorative. Le but de cette étude est donc d’explorer l’état des connaissances sur les pratiques infirmières de suivi de deuil auprès des familles en CHSLD. Une synthèse des écrits a été effectuée selon les principes d’une revue narrative structurée. Un total de 21 écrits a été retenu en fonction des critères de sélection établis. L’analyse thématique de ces écrits a permis de dégager trois thèmes principaux : le suivi de deuil, le rôle de l’infirmière et les interventions infirmières de suivi de deuil. Le suivi de deuil comprend cinq composantes, soit : 1) la reconnaissance du décès, 2) la transmission d’information sur le deuil, 3) l’évaluation du deuil, 4) les interventions de suivi de deuil et 5) l’orientation vers des ressources spécialisées au besoin. L’infirmière est décrite comme une professionnelle occupant un rôle central dans le suivi de deuil en raison de la relation privilégiée qu’elle développe avec les familles et de ses habiletés relationnelles et communicationnelles. Les interventions infirmières de suivi de deuil peuvent pour leur part être regroupées selon quatre types : 1) la tenue d’une rencontre interdisciplinaire de suivi de deuil, 2) une conversation thérapeutique familiale orientée sur les forces, 3) le recours à un outil écrit d’ajustement au deuil et 4) une intervention individuelle de suivi de deuil. Cette étude offre une compréhension approfondie du suivi de deuil ainsi que des pistes d’intervention pouvant être adaptées au contexte d’hébergement. Celles-ci pourraient guider la pratique des infirmières dans ce milieu de soins.
